Making cricket debut at the age of 16, he played for KwaZulu-Natal during the 2006–07 season. Then he played for the Dolphins and played against the Warriors and took 1 wicket for 20 runs on 7 February 2010. Maharaj also played for South Africa Academy in the domestic circuit.

Though he could not make it to the South Africa Under-19 team, later in 2010 he got selected for the South Africa A team and toured Bangladesh in 2010.

Being named in South Africa’s squad at WACA Ground in Perth, he made his test debut on 3 November 2016, playing against Australia. During the match, he scored 16 runs and took 3 wickets for 56 runs in the first innings and scored 41 runs and took 1 wicket for 94 runs in the second innings.

In October 2019, he made his first test fifty by scoring 72 runs in the first innings against India.

On 27 May 2017, he played The Rose Bowl(also known as The Agead Bowl) against England and scored 1 wicket for 72 runs. He made his first 4-wicket haul by taking 4 wickets for 33 runs against Australia on 17 September 2023.

Adding up more, he scored 1 wicket for 19 runs on 10 September 2021 during the match against Sri Lanka at R Premadasa Stadium in Colombo, Sri Lanka.

In September of 2021, Maharaj played against Sri Lanka after being announced as the ODI captain. Also, he was the vice-captain of the team during the 2023 ODI World Cup.

In 2017, Keshav Maharaj was awarded the International Newcomer of the Year at Cricket South Africa’s annual awards.
